you would like to serve as provider of your Personal Care Attendant and Respite Services.
As a recipient of Medicaid Long-Term Personal Care Services (LT-PCS) or Medicare Waiver,
you may report any concerns regarding your services from Premier Community Services to the Bureau of Community Support Services
(BCSS) at 1-800-660-0488.
